  • Family consultation (in-person or by phone) to provide guidance and information on caring for aging seniors

  • Advocacy services, which may include: contacting medical providers and hospital discharge planners; hospital visitations; monitoring at nursing home or assisted living facilty
  • Arranging for outside services, such as: in-home caregiver, housecleaner, handyman, repairman
  • Routine in-home visits to monitor the status of the older adult and oversee any outside services that are provided
  • Referral to community resources, such as: support groups, home health services, financial planning, legal advice, Meals-on-Wheels, senior center, adult day care
  • Coordinate and manage all aspects of a local or long distance move
  • Routine contact with long distance family members via e-mail or phone
  • Oversee bill paying
  • Accompany client to doctor’s visits and promptly report findings to family members
  • Information on Medicare and Medicaid regulations
  • Crisis intervention with care managers available 24/7 to accompany seniors to hospital emergency room
  • Home safety evaluation
  • Secure and oversee contractors to make home handicap accessible
  • Medical claims oversight to prevent overpayment
  • Short-term services for local families on vacation
