  • In-home assessment to determine social needs, functional abilities, assess safety issues, and ascertain which outside services will help an individual remain safely in their home or transition to another level of care
  • Develop a care plan based on the results of the in-home assessment to be used as a guide for further care
  • Family consultation (in-person, by phone, or Zoom) to provide guidance and information on caring for aging seniors
  • Information on local retirement communities, independent living communities, assisted living facilities, and nursing homes, when appropriate
  • Advocacy services, which may include: contacting medical providers and hospital discharge planners; hospital visitations; monitoring at nursing home or assisted living facility
  • Arranging & coordinating outside services, such as: in-home caregiver, housecleaner, handyman, repairman
  • Regular in-home visits to monitor the status of the older adult and oversee any outside services that are provided
  • Referral to community resources, such as: support groups, home health services, financial planning, attorneys, meal delivery, adult day care for memory impairment, prescription delivery

Let’s Get Started

In-Home Assessment

Schedule a meeting to visit with the individual or couple with a goal to review their needs and abilities to determine what is required to help them remain in their home. Or, if they are living in a facility, we’ll meet with them to review care. An outside advocate is very important to make sure that their care needs are being properly met.

Recommend, Coordinate, and Manage

Once their needs are determined then we make care recommendations and work with the family to coordinate and manage the care as needed. Whether the family lives near or far, we are able to assist with oversight of all care being provided and communicate with the family. We also watch for changes in needs based on our experience with the aging process so that you are ahead of the changes.

As much as we try to help an aging person live in their own home as long as possible there are sometimes situations where moving to an assisted environment is ideal. If that is the case, we can help you find the best match for their care needs and can help coordinate the move in a way that affects them as little as possible.

What is a Professional Geriatric Care Manager?

A Geriatric Care Manager, also called an Aging Life Care® Manager, is a specialist who has expertise in issues related to aging and elder care. They work with older adults and their families to develop a plan of care to solve aging issues they are facing. They provide education and share available options. They always operate with compassion, respect, and sensitivity.

Health & Human Services Professional

Degree in Social Work, Psychology, Nursing, Gerontology, Counseling, or Rehabilitation

Works as a Private Family Consultant

Meet with clients and their families to assess the care challenges being faced, create a plan of care, and determine recommendations to solve the issues.

Makes Client Home Visits

Regularly checks on clients to make sure their care needs are being met in all aspects of their life.

Coordinates Medical Care

Whether a client needs assistance at medical appointments, getting tests completed, or is in the hospital for a crisis or other care, their care manager will assist with coordinating care, including discharge planning, and communicating with family.

Recommend Home Based Services

Often a client can live at home longer with independence by having assistance from in-home care agencies. Your care manager helps find qualified care and also monitors care provided in the home to make sure quality is maintained.
